DALTON SURGERY LEARNING DISABILITY CHAMPIONS

Patients, families and carers

Dalton Surgery has nominated 3 members of staff to be our Learning Disability Champions.  Our champions are Dr Rabbani, Angela Robinson, Practice Nurse and Helen Gale, receptionist who are upskilled and knowledgeable in supporting patients with a learning disability.

Helen will be patient’s first point of contact, to contact you with patients preferred communication method and patients reasonable adjustments will be looked at closely.  Angela with the support from Dr Rabbani will carry our patient annual health checks. 

The champions will liaise closely with the Strategic Health/Community Learning Disability Team with regards to any on-going support patients may require.
